 Please read before enrolling
- The online enrollment form is identical to the form in the camp guide (page 16 & 17)
- Please make sure to complete all required fields, including Camper Physician, Emergency Contact, and all necessary authorized signatures -- at bottom of page 1 and with all credit card and Electronic Funds Transfer fields on page 2
- Just as when forms are submitted in person, this is ONLY THE FIRST STEP! Campers spots will be saved once payment plan is processed by the JCC Accounting Department. When payment plan has been processed, your receipt will be emailed. You will be contacted with any questions or concerns.
- Don't worry too much about getting all your math right (oy, the discounts!), we will check to make sure you're charged correctly!
Submitting Online Forms (documents opened in Adobe Reader cannot be saved)
- Download zipped file to your desktop
- Double-click on zipped file
- Complete form and double check all authorization signatures by using Preview from your computer
- Save completed form making sure all fields are correct and entered
- Open email and send completed form AS AN ATTACHMENT to jcckccamp@gmail.com
- You will receive an automated email response that your enrollment form has been received
